PLAYER CONTRACT

(Player) hereby agree to play amateur baseball during the
Summer, 2009 season for the
___________________________(Team) Fort
Bend Baseball League, in accordance with
the regulations of the Fort Bend Baseball Association, unless released in
accordance with those regulations. I
acknowledge that I am not to receive any compensation, direct or indirect, for
playing with this team. In consideration
of being allowed to participate in this league and any activities associated with
Fort Bend Baseball Association and related events and activities, the
undersigned:
1. Acknowledge that Player is physically fit to participate
as a player in this league;
2. Acknowledge and fully understand that Player will be
engaging in activities that involve risk of serious injury, including permanent
disability and death, with the possibility of severe social and economic losses
which might result from Player=s participation or presence during Fort bend Baseball
Association events or activities, including but not limited to Player=s own acts or omissions, the acts and omissions of
others, the rules of play, the condition of the premises, or the use of any
equipment.
3. Assume any and all risks related to Player=s involvement in any events and activities of Fort
Bend Baseball Association and accept personal responsibility for any injury,
damage, permanent disability or death, and
all damages resulting there from.
4. Release, waive, discharge, and
covenant not to sue Fort Bend Baseball Association, its affiliated clubs, their
respective administrators, directors, agents, coaches, employees, or
volunteers, other participants, sponsoring agencies, sponsors, advertisers, and
if applicable, owners and leasors of premises used to conduct any event,
hereinafter AReleased Parties@, from any and all claims,
causes of action, losses, or damages which Player may have or may have in the
future as a result of injuries or damages, sustained or incurred by Player from
whatever cause including but not limited to the negligence, breach of contract
or wrongful conduct of the parties hereby released, and further agree to fully
defend, indemnify, and hold harmless the Released Parties from any and all
claims or causes of action arising out of any injury, death, or damage incurred
by, through, or under Player, specifically including all claims alleging
negligence, in whole or part, of the Released Parties.
